Whistleblower Form

ADA’s policy is to welcome the employees’ efforts that ensures and contributes to the compliance with ethical standards and local legislations. If an employee has knowledge of or a concern of the illegal or fraudulent activity, he/she is encouraged to report on immediate basis if any of the specified actions are happening, have happened or are likely to happen.

 

All employees are encouraged to report any serious concerns in the below form.

 

Any information you have will be useful, especially the following:

  • Identity of wrongdoer
  • Date
  • Location
  • Details of concern

Types of Concerns to be Reported

 This form deals with reporting concerns related to the following areas:

  1. Financial Reporting
  2. Suspected Fraudulent Activity
  3. Breaches of the code, other compliance policies and laws and regulations
  4. Retaliation or retribution against an individual who reports a concern
